This is an application for license to use Playa del Este Upper Private Party Group Area located at 28051 Lanzarote, Mission Viejo, in accordance with the following terms and conditions set forth below. If the application is approved by Lake Mission Viejo Association, it shall become a pending License Agreement between Lake Mission Viejo and applicant upon payment to LMVA of all required fees.

The undersigned (Applicant) hereby applies this day of , 20 , to the Gibraltar School Board or its designee of the Gibraltar School District (School District) for community group use of the Door Community Auditorium, and if this application is granted, in consideration of the granting of the application and other consideration set forth, hereby agrees that this application constitutes a binding Contract between the undersigned and the School District for the use of school facilities.

Your security deposit will be returned to you after an inspection of the Park is completed. Any cost for damage or excessive clean-up will be deducted from the security deposit. Recreation reserves the right to cancel this permit for any date needed for a Township Recreation Event. Violation of the attached rules and regulations could cause permit to be reevaluated or revoked. Organizations/groups/ wedding shall obtain and produce certificate of liability insurance for at least 1 million dollars naming Holmdel Township as additional insured, absolving Holmdel Township, its employees and representatives of all liabilities relative to the use of the requested facility. Carry in-carry out all supplies. Smoking is prohibited. The serving and consumption of alcohol is allowed by special permit. All applicable permits must be secured and approved prior to use. Depending upon the nature of the request, there may be additional fees for staffing & related expenses.
