Your reservation is not complete until the deposit form, deposit, and this reservation form have been received by Xtreme Challenge. This form must be used for ONLY Birthday Parties. Your deposit form is on page 4. If you are booking a different type of event (e.g., youth group, on location, camp days, or corporate team building), please refer to our website for the correct forms.
