SCREENING CONTRACT (the “Contract”) is entered into by and between the Arizona Family Health Partnership, an Arizona not-for-profit corporation (the “Partnership”), and Pinal County Public Health Services District (the “Contractor”). The Partnership or the Contractor may be referred to individually as the “Party” or collectively the “Parties”.
