This Confidentiality and Data Use Agreement (the “Agreement”) is dated as of the date of last signature below (the “Effective Date”), and is between the University of Florida Board of Trustees, a public body corporate of the state of Florida with offices at the UF Research Sponsored Programs, 207 Grinter Hall, Gainesville, FL 32611 (“Institution”) and _____________________, with offices at _____________________ ("Receiving Party”). Institution acts on behalf of the Consortium for Medical Marijuana Clinical Outcomes Research (the “Consortium”), a research consortium of public and private universities in Florida established by Florida Statute 1004.4351, which is located within the Institution.
