The Health and Human Services Commission (“HHSC”), acting for and on behalf of Rusk State Hospital (“RSH”), and Jumah T. Absy, OD dba Healthy Eyes Vision Center, PA (“Contractor”), collectively referred to as the “Parties” to HHSC Contract No. 52900-8-2000176810 effective September 1, 2017 (“Contract”), now want to amend the Contract further. Under the terms of the Contract, Contractor will provide optometry services (“Services”) to RSH patients.

Box 201301, 5 South Last Chance Gulch, Helena, MT 59620-1301, 406-444-3930 and Eye Clinic of Great Falls, (CONTRACTOR), whose address and phone number are 509 2nd Avenue North, Great Falls, MT 59401 and (406) 452-2015.
