This agreement will allow the Michigan Data Collaborative, a division of the University of Michigan, to combine patient identifiable data from providers and multiple payers, then share back the data to your practice or Physician Organization. As a vendor to MDHHS, the MDC will aggregate patient identifiable data and create measures for the SIM PCMH Initiative. Only your own patient identifiable data will be available for access or delivery to the Provider or Physician Organization. Some measures will reflect a population of combined de-identified data. This Data Sharing and Use Agreement includes all relevant HIPAA requirements and obligations to keep data confidential and secure.
