This California Reportable Disease Information Exchange (CalREDIE) Data Use And Disclosure Agreement (hereinafter referred to as “Agreement”) sets forth the information privacy and security requirements that Humboldt County Health Department (hereinafter “Data Recipient”) is obligated to follow with respect to all CalREDIE System Data, and other personal and confidential information, (as each of these types of data and information are defined herein), disclosed to Data Recipient by the California Department of Public Health (hereinafter “CDPH”). (Such CalREDIE System Data and other personal and confidential information are also referred to herein collectively as “Protected Data”.) This Agreement covers Protected Data in any medium (paper, electronic, oral) the Protected Data exist in.
