This Contract Number MA-042-24011105 (“Contract”) is made and entered into this 1st day of July, 2024 (“Effective Date”) between Chorus Innovations, Inc. (“Contractor"), with a place of business at 4265 E. Conant Street, Suite 201, Long Beach CA 90808-1968, and County of Orange, a political subdivision of the State of California ("County”), through its Health Care Agency with a place of business at 405 West 5th Street, Santa Ana CA 92701. Contractor and County may sometimes be referred to hereinafter individually as “Party” or collectively as “Parties”.
