This Master Large Group Contract, together with the documents described in Section 2.1 below (collectively, the “Contract”), is entered into between Samaritan Health Plans, Inc., an Oregon nonprofit corporation (“SHP”), and (the “Group). This Contract is effective as of the date prescribed in Section 5 herein.
