The Health and Human Services Commission (“System Agency”) and Collin County MHMR Center dba Lifepath Systems (“Grantee”), each a Party and collectively the “Parties” to that certain Recovery Support Services for Community-Based Organizations (RSS/CBO) agreement denominated HHSC Contract No. HHS000779800014, effective September 1, 2020 (the “Contract”), now want to amend the Contract.
