(“FIL”), and agreed and acknowledged by the principal investigator listed below (“Principal Investigator”). This Agreement consists of this signature page, the general terms and conditions attached hereto (“Terms and Conditions”), and any schedules attached to the Terms and Conditions. The signature date of the last party hereto to sign this Agreement will be this Agreement’s effective date (“Effective Date”).
