The APPLICATION AND POLICY SCHEDULE FOR STOP LOSS COVERAGE attached hereto and made a part of this Policy (herein called the “Stop Loss Application”) shall establish the Policyholder’s name, Group Number, State of Situs, the Effective Date of Policy, and the Policy Period.

The Application for Stop Loss Coverage (herein called the “Application”) attached hereto and made a part of this Policy shall establish the Policyholder’s Employer Group Name, Employer Group Number, the Effective Date of Policy, and the Policy Period.
