This Student Experience Agreement is between Iowa State University of Science and Technology (“ISU”) and (“Monrovia Unified School District”). The purpose of this agreement is to provide supervised practical experiences (which may be clinical or non‐clinical) at Facility to students enrolled at ISU in the mutually agreed upon programs listed in Exhibit A.

This contract letter serves as confirmation that School Board of Sarasota County, Florida and STATE COLLEGE OF FLORIDA have reviewed their Student Experience Agreement for the period of July 1, 2012 through June 30, 2016 and hereby affirm the same for the renewal year commencing July 1, 2016 and ending June 30, 2017, subject to the terms thereof for our, Health Professions Programs. Please have your administrator sign and keep a copy for your files and return the other to us either via email or postal service.

This Agreement is made and entered into this 18th day of September 2018, effective as of September 1, 2018, (the “Effective Date”), by and between THE DISTRICT BOARD OF TRUSTEES OF STATE COLLEGE OF FLORIDA, MANATEE-SARASOTA, (hereinafter referred to as "College"), and the SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA (hereinafter referred to as "Educational Facility"), located at 1960 Landings Boulevard, Sarasota, FL 34231.
