Welcome and thank you for using the TriVita VitalityPlanner, (“Service”) a TriVita, Inc. service, powered by BaseHealth. With our Agreement, the words, “you” or “your” mean the person who will receive Services (defined below) and who uses this science-validated predictive wellness platform. The Agreement is made among the User and TriVita and each a “Party” and together, the “Parties.” The “Effective Date” will be the date on which the User first agrees to the User/Consent Agreement. This Site incorporates proprietary intellectual properties, and is maintained by TriVita and/or its affiliates, as confidential and proprietary information to be used only in connection with this User/Consent Agreement.
