Goal Setting Form. The Goal Setting Form is intended for collaborative use between administrator evaluating and teacher needing further development. The Goal Setting Form may be used after any observation or after an evaluation in lieu of a Personal Development Plan if all the following conditions apply:
Goal Setting Form. A form is provided to help the teacher and evaluator with the goal setting process. It helps the teacher in monitoring goals and informing mid-and end-of-year conferences. High Impact Guide (HIG): This guide provides an action plan for improvement in student outcomes that rests on the premise that if we are to make a difference for all students, we need to move from “intention” to “evidence.” The HIG embodies the essence of the teaching and learning plan in District 438; it establishes the common language. Informal Observation: Informal Observation is defined as an observation by a qualified evaluator that is not announced in advance of the observation and not subject to a minimum time requirement. Informal observations must be documented in writing (either electronic or paper) to be used in Formal Conference meetings and summative rating. If desired, it is the responsibility of the staff member to schedule a meeting after an informal observation with the evaluator. Examples of Goal Setting Form in a sentence

  • Beginning with the 2014-15 school year, a TLE Goal Setting Form may be used after any observation or after an evaluation, in lieu of a PDP when the teacher has no more than 2 ratings of 2–Needs Improvement and has no rating of a 1- Ineffective.

  • When the evaluation process of a teacher indicates the need to improve professional skills, the evaluating administrator shall provide a detailed plan to eliminate difficulties noted on the observation and/or evaluation through a Goal Setting Form and/or Personal Development Plan.

  • The evaluator shall schedule an annual goal-setting conference to meet and discuss the 112 employee’s Professional Reflection and Goal Setting Form.

  • If use of the Goal Setting Form is not successful at the evaluation stage, then a Personal Development Plan is warranted.

  • When the evaluation process of a Member indicates the need to improve professional skills, the evaluating administrator shall provide a detailed plan to eliminate difficulties noted on the observation and/or evaluation through a Goal Setting Form and/or Personal Development Plan.

  • Unit members shall review, revise, and resubmit the FESD Certificated Goal Setting Form to their supervisor by June 1.
