Examples of Grievance Review Request Form in a sentence

  • In formally presenting a grievance at Step One or Step Two, a Grievance Review Request Form addressed to the appropriate administrator will be submitted.

  • A copy of the Grievance Review Request Form also shall be sent to the Assistant Superintendent of Human Resources.

  • Within three (3) days after the receipt of the Grievance Review Request Form (Appendix A – Form B), the Superintendent will either accept responsibility for revising the grievance or assign another District administrator to review the grievance.
