Common use of Accident and Sickness Clause in Contracts

Accident and Sickness. An employee who is absent from work over twenty-four (24) hours shall provide the Fire Chief with a certificate from a physician within ten (10) calendar days from the commencement of sickness or immediately upon return to work, whichever occurs first, reporting the duration or probable duration of the sickness with the first and most recent dates of attendance upon the employee, and the expected date of return to work.
