Accident and Sickness. 24:01 Sick Leave means the period of time an employee is permitted to be absent from work by virtue of being sick, disabled, quarantined, or because of a non-work related accident not covered under the Workplace Safety and Insurance Act. 24:02 Sick Leave shall not mean any time absent from work due to an accident or disablement incurred, while employed by an employer other than the City of Guelph, where compensation for that accident or disablement is being paid under the Workplace Safety and Insurance Act through the other employer. 24:03 Notification Requirements for Absence: (a) Employees who are unable to assume their normal duties on any scheduled working day must notify their supervisor prior to their normal starting time. If the employee leaves the employment of the City, prior to repayment of the overpayment in full, the amount shall be deducted from their final pay. (g) Pension contributions, union dues, and all required deductions shall be deducted while the employee is on STD. If the employee is receiving seventy-five percent (75%) STD with no top-up, they would be required to sign an agreement to have a reserve established to buy back OMERS, on return to work. (h) Where the City or the Insurance Carrier requires the employee to supply a medical certificate to verify sick leave, the City agrees to reimburse the employee for one hundred (100%) of the cost of the medical certificate(s), upon submission of a receipt(s).

(4) the Workplace Safety and Insurance Board awards a permanent total or permanent partial disability benefit;
(5) employment terminates; or
(6) the employee takes an unreduced retirement pension from ▇▇▇▇▇.
(a) All regular employees shall have placed to their credit, one and one-half (1- 1/2) days sick leave for each full month of employment, but are not entitled to draw sick leave pay until they have accumulated three (3) months service with the Corporation.
(b) Any employee absent through his or her own illness shall notify the Department of their inability to perform his or her duties due to sickness at the earliest opportunity and in any event not later than the commencement of the first shift of absence.
(c) An employee who is absent from work by virtue for three (3) consecutive working days shall provide the Fire Chief with a certificate from a physician within seven (7) calendar days from the commencement of being sicksickness or upon return to work, disabledwhichever occurs first, quarantinedreporting the duration or probable duration of the sickness with the first and most recent dates of attendance upon the employee, or because and the expected date of a non-work related accident return to work.
(d) An employee shall not covered receive sick pay when eligible to receive compensation under the Workplace Safety and Insurance Act.

9.03 Each employee absent on sick leave for a period of less than three (3) days and after the third (3rd) time in discussions regarding their claim each calendar year may be required, at the discretion of the Fire Chief, to produce a physician’s certificate.
9.04 If an employee is absent on sick leave more than once on a holiday recognized in Article 8.01 (a), within the preceding two (2) years, the Chief, at his discretion, may request a physician’s note. If a note is requested under this clause, this absence counts toward the total three (3) absences before a note is required in Article 9.03.
9.05 An employee absent for more than fifteen (15) consecutive working days shall furnish immediately following such fifteen (15) days, and each subsequent fifteen (15) consecutive days of absence, a certificate from the employee’s personal physician stating latest date of attendance and the probable date on which the employee does have grievance rights under Article 12:01, at Step 2will return to duty.
9.06 An employee absent from work in excess of sixty (f60) Should consecutive calendar days for medical reasons, will, prior to returning to work, be required to have a medical assessment regarding the STD claim condition which caused the absence, by a physician selected by the Corporation. An employee, who is not satisfied with such assessment, will have the right to be denied after exhausting examined by a physician of the above optionsemployee’s choosing. If the report of the employee’s physician is contrary to the first report, the employee will be examined by a third physician satisfactory to both parties. The Corporation will underwrite the cost of all such medical examinations.
9.07 Where an employee is absent on account of illness and cumulative sick pay credits have been exhausted, the employee shall not receive sick pay credit for the month of absence. Where an employee is absent for more than six (6) months due to illness or injury, sick leave credit shall not further accumulate during the absence.
9.08 An employee shall not be required entitled to repay sick pay in advance of any overpayment monies credit earned in the current month; such credit becomes available only on and after the first day of the following month.
9.09 Whenever in any month an employee’s days of illness exceeds the cumulative sick pay credit at ten percent (10%) per the end of such month, the excess days of illness shall not be charged against credit becoming available in the future, but shall be regarded as days of illness without pay. If .
9.10 When an employee is given leave of absence without pay for any reason, or is laid off on account of lack of work and returns to the service of the Corporation upon expiration of such leave of absence, the employee leaves shall not receive credit for the employment period of such absence, but shall retain the Citycumulative credit, prior to repayment if any, existing at the time of the overpayment in full, the amount such leave.
9.11 The number of days or parts of days for which an employee received "Sick Pay" shall be deducted from their final paythe employee’s Cumulative Sick Pay Credit. If twenty-four (24) hour shifts are worked, then twenty-four (24) hours shall be considered as two (2) days.
(g) Pension contributions9.12 Any or all the unused portion of Sick Pay Credit shall be accumulated to the benefit of the employee from year to year. The unused portion of the yearly accumulation shall be computed at the end of each year and brought forward in days.
9.13 Where an employee who is injured in circumstances which might entitle the employee to compensation under the Workplace Safety and Insurance Act, union dueselects instead to claim against the third person, the employee shall, as a condition of receiving sick pay benefits as hereinbefore provided, undertake in writing to reimburse the Corporation out of the proceeds of any settlement or judgement upon such claim the amount of money equivalent to the value of such sick pay benefits, and all required deductions upon having made such reimbursement, the employee’s accumulated sick pay credits shall be deducted restored accordingly.
9.14 An employee who is injured on duty where no action for such injuries would lie against a third person, and who is unable to work as a result of such injury, shall, while off work, be advanced by the Corporation to the extent of available sick leave credits an amount equal to an estimate of the anticipated WSIB daily benefit to which the employee is on STDmay be entitled, until such time as a ruling has been made by the Board upon the employee’s claim. If the Board rules against the claim, the accumulated sick leave credits of the employee is receiving seventy-five percent (75%) STD with no top-up, they would shall be required to sign an agreement to have a reserve established to buy back OMERS, on return to workreduced accordingly.
(h) Where 9.15 This section applies only to those employees who were hired in the City or the Insurance Carrier requires the employee Fire Department prior to supply a medical certificate to verify sick leaveOctober 7, the City agrees to reimburse the employee for one hundred (100%) of the cost of the medical certificate(s), upon submission of a receipt(s)1987.

(i) The waiting period for STD coverage will be from 1st day of accident or from 1st day of hospitalization or from the 3rd day of sickness.
(ii) STD will shall be paid to a maximum of twenty-six (26) weeks.
(iii) The STD benefit will be based on seventy-five percent (75%) the employee's executors or administrators, and shall form part of the employee’s standard base weekly pay's estate. Standard weekly pay is the weekly hours of work times the employee’s rate of pay at the time of
24:05 Applying for Short Term Disability (STD):
(a) An Employee applying for STD benefits is required to complete the STD forms of the Insurer, available from the Human Resources Department.
(b) The waiting period for illness, would be Days deducted from the employee’s non-cumulative sick hours. If the employee does not have sufficient non-cumulative sick hours, the employee may request to be paid from their stat lieu bank, banked overtime, floater bank, vacation or personal days.
(c) Employees applying for STD benefits shall receive up to four (4) weeks payment, while waiting on the adjudication by the Insurance Carrier. If the claim is denied, the employee may appeal the decision to the Insurance Carrier.
(d) If an employee has cumulative sick leave hours they would be used until depleted credits will apply to top-up STD to one hundred percent (100%)working days only. If the employee has no cumulative sick leave hours, the employee may request to top- up their STD with non-cumulative sick hours, stat lieu bank, banked overtime, floater bank, vacation or personal days.
(e) An employee The Employer shall be accompanied by the designated Union Representative in discussions regarding their claim and the employee does have grievance rights under Article 12:01, at Step 2.
(f) Should the STD claim be denied after exhausting the above options, the employee shall be required to repay any overpayment monies at ten percent (10%) per pay. If the employee leaves the employment of the City, prior to repayment of the overpayment in full, the amount shall be deducted from their final pay.
(g) Pension contributions, union dues, and all required deductions shall be deducted while the employee is on STD. If the employee is receiving seventy-five percent (75%) STD with no top-up, they would be required to sign an agreement to have a reserve established to buy back OMERS, on return to work.
(h) Where the City or the Insurance Carrier requires the employee to supply a medical certificate to verify sick leave, the City agrees to reimburse the employee for one hundred (100%) assume of the cost of the medical certificate(s), upon submission premiums of a receipt(s).the following health coverage benefits:
