Activity Coordinator. Contractor shall provide 2 FTE Activity Coordinator positions for activity development, implementation, training, oversight, and support. Contractor’s Activity Coordinator(s) must be On-Site a minimum of 7 days per week. Responsibilities include ensuring direct care staff are trained on the Activity Plan, and that Individuals can participate in activities 7 days per week, even if an Activity Coordinator is not On-Site or Available. Activity Coordinator shall: a. Conduct a written assessment for each Individual that addresses, at a minimum, the following: (1) Past and current interests; (2) Current abilities, skills and interests; (3) Emotional and social needs and patterns; (4) Adaptations necessary for the Individual to participate; and (5) Identification of activities needed to supplement the Individual’s Behavior Plan. b. Develop an Activity Plan for each Individual within 15 business days of admission, based on the Activity assessment. The resulting Activity Plan must meet the preferences of each Individual and be Available on day and evening shifts, 7 days per week. Activities shall include scheduled or planned as well as spontaneous activities, and which are collaborative and support the Behavior Plan. Activities may include, but are not limited to: (1) One-to-one activities that encourage positive relationships between Individuals and Contractor’s staff (e.g. life story, reminiscing, music); (2) Spiritual, creative, and intellectual activities; (3) Sensory stimulation activities; (4) Physical activities that enhance or maintain an Individual’s ability to ambulate or move; and (5) Outdoor activities c. Be reviewed each month, and as needed, by Contractor’s Activity Coordinator and modified, as needed, based on feedback from direct care staff, SPT and the Individual’s responses; and d. Provide training needed to Contractor’s direct care staff to implement current Activity Plans.
Activity Coordinator. Contractor’s Activity Coordinator shall engage CMHP staff in the development of Individualized Activity Plans, ensuring alignment with the Behavior Plan. Activity Plans shall support skills building, as outlined by the Behavioral Health Treatment Plan and reviewed during the IDT. Coordination between the Activity Coordinator and CMHP staff must be addressed in the Memorandum of Understanding, to ensure provision of required training for Certified Nursing Aides to implement current Activity Plans 24/7. Contractor shall provide 1 FTE Registered Nurse (RN) assigned to the Individuals served under this contract on day and evening shifts. Nursing duties on night shift may be provided by an RN assigned to the overall facility. The night shift nurses who work on the Individuals served under this contract must receive orientation to the unit and participate in relevant shift reports. Contractor’s RN’s shall perform the following tasks:

Activity Coordinator must develop, oversee and provide activities required under Oregon Administrative Rules Chapter 411, Division 054 rules and as described in this Contract. Activity Coordinator. 16.18.1 Each comprehensive high school shall be staffed with a minimum .4 FTE release activity coordinator with the extended days identified in Section 15.16 and the stipend identified in Schedule 23SH.
Activity Coordinator. The District will coordinate opportunities for Activity Coordinators to collaborate with job alike peers. The District will communicate a clear outline of the roles and responsibilities of Activity Coordinators. Such appointment shall be made from a list of recommendation(s) made by class/club sponsors who will have a direct working relationship with the Activity Coordinator. (If the principal requests, the list shall consist of at least three (3) names.) Such appointment and recommendation(s) shall take into consideration prior expression of interest of such service and an assessment of the abilities of the person(s) expressing such interest to perform the requisite duties. After the principal has notified the sponsors of the upcoming appointment, the sponsors shall have ten (10) work days to submit their recommendation(s) for the position.

Activity Coordinator. Contractor must provide a minimum of 60 hours per month by a designated employee who is the Activity Coordinator. This position must ensure that caregivers are trained on and are implementing person- centered activity plans so that Clients can participate in ‘on and off site’ activities 7 days a week as indicated on their individual activity plan. This position will provide services and tasks defined under OAR 411-050 and as described in this Contract. Activity Coordinator is responsible for: (1) Conducting a person-centered activity evaluation completed at admission for each Client that addresses the following: (a) Past and current interests; (b) Current abilities and skills; (c) Emotional and social needs and patterns; (d) Physical abilities and limitations; and (e) Adaptions necessary for the Client to participate. (2) Development of a person-centered Activity Plan as part of the initial Care Plan. The Activity Plan must include structured and non- structured activities which meet the preferences of each Client which can be provided on day and evening shifts, seven days per week. Activities may include but are not limited to: (a) Occupation or chore related tasks; (b) Scheduled and planned events (e.g. entertainment, outings); (c) Spontaneous activities for enjoyment or those that may help diffuse a behavior; (d) One to one activity that encourage positive relationships between Clients and staff (e.g.) life story, reminiscing, music (e) Spiritual, creative, and intellectual activities; (f) Sensory stimulation activities; (g) Physical activities that enhance or maintain a Client’s ability to ambulate or move; and (h) Outdoor activities. (3) Reviewing and updating each Client’s plan on a monthly basis or more frequently if Clients refuse participation, their condition changes and/or they are referred for Behavior Support Services. Monthly reviews must be documented. (4) Providing training to caregivers and Activity Aide(s) on a quarterly basis on how to implement current Activity Plans. (5) Staff designated to implement Activity Plans shall document time spent, type of activity and Client response on a daily activity log that is monitored weekly by the Activity Director.
