Applying for Leave. All leave whether it is to be paid or unpaid requires the prior approval of the employee’s Supervisor and the agreement of the relevant Manager. All leave will need to be planned to fit in with service requirements. However, approval will normally be given if the workload can be satisfactorily covered during the period of leave. Paid leave must be taken before any unpaid leave will be granted. Where practicable, two weeks’ notice of an employee’s intention to take annual leave must be given to the line manager.
Applying for Leave a.) an employee’s application for a leave of absence must be made via telephone to the designated third party administrator (TPA) thirty (30) calendar days in advance of the leave, except in cases of emergency; b.) in cases of emergency, the employee shall contact his/her manager and explain the circumstances requiring emergency leave and must notify the TPA as soon as possible. The leave will either be approved or not approved; the employee must complete the appropriate paperwork within seventy-two (72) hours of the time the leave is approved; c.) the employee’s application must include the beginning and end dates of the leave, with statement of the employee’s intent to return to work;
Applying for Leave. Faculty shall have the right to apply for any of the various forms of leave as detailed in University policy and all applicable laws. Absent extraordinary circumstances, any leave or combination of leaves shall not exceed two (2) consecutive academic years. When applying for leave, Faculty shall provide full and accurate information. The University Administration shall evaluate each application on its own merits, giving due consideration to the needs and priorities of both the individual and the institution. A timely response to each request for leave shall be provided in writing to the applicant.

Applying for Leave. 10.2.1 The taking of Leave is to be determined by mutual agreement between the employee and ▇▇▇▇▇ ▇▇▇▇▇. 10.2.2 ▇▇▇▇▇ ▇▇▇▇▇ shall respond to annual leave applications within 14 days. 10.2.3 ▇▇▇▇▇ ▇▇▇▇▇ shall have regard to the leave arrangements of an employee’s partner when considering leave requests. 10.2.4 Annual leave will not generally be granted at the busy times of the year, for example: Christmas and Clearance, and for specific departments leave will be limited at other times, for example, confectionery near Mothers Day and Valentines Day.
Applying for Leave. 14.2.1 Annual leave may be taken for a period agreed between ▇▇▇▇▇ ▇▇▇▇▇ and the employee. ▇▇▇▇▇ ▇▇▇▇▇ will not unreasonably refuse to agree to a request by the employee to take paid annual leave. 14.2.2 ▇▇▇▇▇ ▇▇▇▇▇ shall respond to annual leave applications within 14 days. 14.2.3 ▇▇▇▇▇ ▇▇▇▇▇ shall have regard to the leave arrangements of an employee’s partner when considering leave requests. 14.2.4 ▇▇▇▇▇ ▇▇▇▇▇ and employees appreciate that granting of annual leave is challenging during key operational periods, which include the Christmas and Clearance trading period. Any requests made during this time will not be unreasonably refused and will be considered on a case by case basis.

Failure to arrange those deductions in advance of the leave, will be cause for the Employer/Hospital to terminate the benefits during the leave; and
Applying for Leave. When reviewing requests for leave, ▇▇▇▇▇ ▇▇▇▇▇ will include consideration of school holidays. ▇▇▇▇▇ ▇▇▇▇▇ will continue to offer training to part-time and casual employees pursuant to the provisions of the ▇▇▇▇▇ ▇▇▇▇▇ Enterprise Agreement 2012 (“the Agreement”). However, due to the nature and duration of some training modules, ▇▇▇▇▇ ▇▇▇▇▇ may offer such employees the option to undertake training online (“Online Training”) at the store or away from the store. When an employee, at their convenience, chooses to undertake training modules online and away from the store, eg, at home, they may undertake such training in blocks of less than 2 hours. Online training may be conducted in blocks of less than 2 hours and managed in the relevant pay cycle on the proviso that within a 12 month period any training equates to no less than 2 hours, or alternatively the employee is paid in accordance with the relevant minimum call. The minimum training engagement will apply for a maximum of 6 training engagements in any one year as per clause 7.4.1 of the Agreement. Where and employee does not elect to undertake Online Training away from their store, ▇▇▇▇▇ ▇▇▇▇▇ will ensure that employees will be able to complete Online Training at their store. Any Online Training sessions undertaken at the store will be pursuant to the provisions of the Agreement. ▇▇▇▇▇ ▇▇▇▇▇ will offer support to employees who may require assistance in undertaking Online Training or who elect to undertake training in store. Employees taking their LSL as double time at half pay may elect to receive a full pay every second fortnight (the options of pay in advance or pay each fortnight remain).
Applying for Leave. 13.1. All applications for Personal leave must be submitted on a Leave Application Form. 13.2. Applications for personal leave must be accompanied by supporting documents in accordance with this protocol and the provisions of the Agreement and the Award. 13.3. Unless specified otherwise in this protocol, leave applications will be approved by the employee’s direct manager/supervisor. 13.4. Where an application for leave is refused for any reason Council will ensure that the reason(s) for the refusal are discussed with the employee.
