Becoming a Member. 2.1 An individual personal customer (not a trader, body corporate – i.e. company – or business) may become a member of the Program by completing and returning this application form and being issued with a membership card (Card).
2.2 In applying for and using the Card the Customer Member agrees to be bound by these terms and conditions, as varied from time to time, and any terms and conditions issued by the Store (available on request).
2.3 The Store may refuse to accept a person as a Member of the Program at its discretion.

Becoming a Member. To become a member of the Credit Union, you must complete a Membership Agreement and open a Primary Savings Account. The basic share deposit that you must always keep in your Primary Savings Account to retain membership is $5.00. There is also a one-time membership fee of $1.00.
Becoming a Member. If you wish to become a member of the Club you will need to complete and submit a New Member Form, make any payments set out within the New Member Form and set up your direct debit (if you are paying by direct debit) and undergo a health check and induction (see clause 3 below). You may need to pay an activation fee which will be set out in your New Member Form. In order to be a member you must be at least 18 years of age. Please check that the details on the New Member Form are complete and accurate, before you sign and submit the New Member Form. If you think that there is a mistake or require any changes, please contact us to discuss. We will confirm any changes in writing to avoid any confusion between you and us. When you sign and submit the New Member Form to us, this does not mean we have accepted your application for membership. Our acceptance will take place when we provide you with written confirmation that your application has been accepted, at which point the terms of the New Member Form and this Agreement will become binding on you and the Contract will come into existence between you and us. If we are unable to accept your application for membership we will inform you of this in writing. We are under no obligation to accept your application for membership. If we accept your application we will:
(a) assign a membership number to you and you should quote this number in all subsequent correspondence with us regarding your membership and when booking personal training sessions; and
(b) provide you with a keypad code or swipecard in order to gain access to the Facilities. Any swipecards provided will remain our property at all times. Upon acceptance of your application for membership, you will be entitled to all of the rights and privileges exercisable by members holding the class of membership for which you have been accepted. Where there is any conflict in or between the terms of this Agreement or the New Member Form, the New Member Form will take precedence.
Becoming a Member. If you are not already a member of DFMC, you must immediately become a member after signing this document and subscribe for 500 shares at $1 per share.
Becoming a Member. An organization within the field of membership of the Credit Union may establish an account under the terms of this Agreement. The organization must provide the Credit Union with resolutions authorizing the opening of the account, in a form acceptable to the Credit Union. The organization must then complete a Business/Other Entity Membership Card. Only persons authorized by resolution of the organization’s governing body will be permitted to make withdrawals from, or give instructions about, the account. The organization must promptly inform the Credit Union in writing of changes in the persons authorized by the organization to make transactions in the account. Until written notice of a change is received by the Credit Union, the Credit Union may rely on the resolution designating persons authorized to transact business at the Credit Union last received from the organization.

Becoming a Member. A person who wants an account must become a member (or the legal guardian acting for a person under the age of 16, or in England, Wales and Northern Ireland 18 if not in employment, who is to be a member) or substitute member and must complete an application form, as required by the scheme administrator and the trustee and make the declarations required by the Relief at Source Regulations.

If your Business Share Account balance drops below the $25.00 minimum membership deposit requirement and is not replenished within forty-five (45) calendar days, as provided in the Credit Union’s Bylaws, your account will be charged a Membership Termination Fee equal to your account balance at that time and your membership will be terminated.
