Claims Conditions. a. In the event you incur a loss you must notify us by providing the following: i. A completed claim form with us as soon as practicable; but not later than one hundred eighty (180) days after the first date of treatment. Claims filed after 180 days will be denied. ii. Invoices from your treating veterinarian listing the services performed, products provided and the itemized charges for treatment, including packages and/or discounts.
