Credentialing Services i. Teach For America shall facilitate the enrollment of individual Teachers in an alternative certification/licensure program that will enable the individual Teacher to obtain appropriate credentials to be a classroom teacher of record. ii. Individual Teachers are responsible for completing all credential requirements, including required coursework through an alternative licensure program. iii. Teach For America shall not be responsible for, and shall not be in breach of any provision of this Agreement, in the event of any failure by an individual Teacher to fulfill his/her obligations to maintain his/her teaching credentials.

ADMINISTRATOR may require, and INTERMEDIARY has agreed, if requested by 16 ADMINISTRATOR, to provide at an additional cost to COUNTY, verification of credentials or 17 “credentialing” of All Providers of Medical Services for the MSI Program. 18 2. Credentialing of all current and new Providers, if required, shall be done at a frequency 19 required by Department, in accordance with the STCs. ADMINISTRATOR anticipates credentialing 20 shall be completed at least once during the term of the Agreement for all current and new Providers 21 determined to not already be credentialed by another entity as approved by ADMINISTRATOR. 22 3. Credentials to be verified shall be as required by the STCs and/or Department, and may 23 include, but not necessarily be limited to: 24 a. State licensure 25 b. Education, training, and board certifications 26 c. DEA/CDS certification 27 d. National Practitioner Data Bank 28 e. OIG sanctions 29 f. Malpractice claims history

Teach For America shall not be responsible for, and shall not be in breach of any provision of this Agreement, in the event of any failure by an individual Teacher to fulfill his/her obligations to maintain his/her teaching credentials.

Individual Teachers are responsible for completing all credential requirements, including required coursework through an alternative licensure program. iii. Teach For America shall not be responsible for, and shall not be in breach of any provision of this Agreement, in the event of any failure by an individual Teacher to fulfill his/her obligations to maintain his/her teaching credentials.

In such an instance, the Teacher shall be afforded the same salary and benefits as a Teacher who possesses the appropriate credentials for the teaching position.

CureMD shall provide the following Credentialing Services (“Credentialing Services”): i. submit and verify receipt of the credentialing application to designated health plans; ii. follow up on the application with designated health plans; and iii. document receipt of requested information by designated health plan, iv. conduct follow-up activities, document acceptance or rejection information from the plan. b. Credentialing Services include submission of additional information requested by designated health plans during the credentialing and enrollment process; however, this does not include submission of different health plan applications for same provider or new applications for other providers. c. CureMD shall be under no obligation to perform any additional services.

  • Credentialing Firm shall be required to access Citizens’ online vendor credentialing system (“CAIS”) to input, update and maintain certain information about Firm and the persons who will perform work related to this Agreement (“Staff”), as provided below and in Exhibit B attached hereto.

  • Training Services All training services, including any Deliverables, are provided for Customer’s internal training purposes only. Customer may not replicate the Deliverables or use the Deliverables to develop any of the products described in such training Deliverables. Training Deliverables are not subject to any maintenance, support, or updates.

  • Counseling Services Therapy is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in mental health counseling, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your therapist, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections. Mental Health Counseling has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness and helplessness, because the process of psychotherapy often requires discussing the unpleasant aspects of your life. However, psychotherapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Psychotherapy requires a very active effort on your part. In order to be most successful, you will have to work on things we discuss in treatment outside of sessions, as a kind of homework. The first 2 sessions will involve a comprehensive evaluation of your needs. By the end of the evaluation, I will be able to offer you some initial impressions of what our work might include. At that point, we will discuss your treatment goals and create an initial treatment plan. You should evaluate this information and make your own assessment about whether you feel comfortable working with me. If you have questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another mental health professional for a second opinion or to make a move to another therapist that feels like a better fit.

  • Billing Services 6 SECTION 3.01.

  • Hosting Services NCR Voyix shall furnish facilities, equipment, computer programs and services, as specified from time to time by NCR Voyix, that NCR Voyix deems necessary for operation and maintenance of the System (collectively, the “Hosting Services”).
