Verification of Credentials. Upon the request of the University, Appointee agrees to have sent to the University, upon its request, certified copies of any transcript(s) reflecting the award of degree(s) listed as received on the Appointee’s curriculum vitae. Appointee further agrees to provide to the University, upon its request, evidence of employability as required by the United States Immigration Laws. Appointee agrees that the employment of Appointee is terminable by the University if at any time Appointee fails to provide such evidence.
Verification of Credentials. Required credentials will vary by specialty, level of expertise, school district, and/or state. Our Compliance Department will follow necessary regulations to verify that each SSG clinician meets the requirements to perform services in the District. Any state licensure, state certification, national certification, or professional certification for our SSG clinicians is confirmed to be Active and in Good Standing through the issuing agency. Credentials of our SSG staff are validated directly through issuing agencies using their online database and copies of each are maintained securely within our HR Department for reference. Ongoing monitoring is managed by validating updated license or certification records at each expiration/ renewal period. Our Compliance Department also provides proactive reminders to our staff well in advance of any credential expiration to prevent any lapse in their license or certification.
Verification of Credentials. Except as set forth in Section 3.35 of the Company Disclosure Schedule, the Company has implemented policies and procedures to verify the credentials (including, but not limited to, with respect to education and licensure) of personnel that the Company places with its clients and to collect, maintain and update such credentialing information. To the Knowledge of the Company, employees, contractors and consultants each consistently follow and have followed such policies and procedures.
Verification of Credentials. Group will cooperate with The Plan or other entity to which The Plan has delegated responsibility for credentialing, in the initial and ongoing verification of credentials of individuals employed by and/or contracted by Group who will be providing Covered Services under the terms of this Agreement. Notwithstanding anything in this Agreement to the contrary, credentialing-related correspondence, including notices of termination for failure to recredential, will be sent to the credentialing address on the Group Participating Provider’s individual credentialing application or the CAQH attestation received by The Plan, whichever is the most recent. Group will report all arrests, criminal actions, disciplinary actions, changes in participation in Medicare or Medicaid programs, changes in admitting privileges and professional licensure of Group Participating Providers, and any changes to the information submitted on Group Participating Provider’s initial or recredentialing application to The Plan in writing within ten (10) days of the action. Group further agrees to ensure that all employees and contracted staff who provide direct patient care maintain current licensure and certification. Group shall allow appropriate representatives of The Plan, or other entity to which The Plan has delegated responsibility for credentialing, access to such documentation upon reasonable request.
Verification of Credentials. Contractor hereby represents and warrants to VL/MMC and to the Clients that the specifics of the resume and other employment, education, licensing, certification, bar admission information and similar credentials as provided by Contractor to VL/MMC is complete and accurate in all respects and constitutes a material portion of the inducement to VL/MMC to enter into this Agreement and will be a material element of the inducement of VL/MMC’s Clients to agree to engage the services of VL/MMC and Contractor. Contractor agrees to indemnify and hold VL/MMC harmless from all claims alleged against or damages suffered by MMC which are attributable in part or in whole to any omissions or inaccuracies as contained in any such information. As applicable, contractors who are attorneys represents and warrants that he or she is eligible to practice law and is in good standing with the respective state Bar in which he or she intends to practice. In the event Contractor is sanctioned, reprimanded, or in any way disciplined by any state Bar association or becomes ineligible to practice law for any reason he or she agrees to immediately notify VL/MMC and not to accept or perform any additional work for VL/MMC or VL/MMC’s clients
Verification of Credentials. You acknowledge and agree to CloudLaw’s use of the following procedures to verify Your credentials:
a. In the case of Your initial registration on the Site, We require You to provide us with information and/or evidence regarding Your license to practice law in the jurisdiction(s) in which You purport to be licensed, including the year You obtained Your license in each jurisdiction, and Your attorney registration or identification number, if one has been issued to You, and Your status in secondary jurisdictions.
b. We will use information available from the jurisdiction(s) in which You purport to be licensed to determine if there is, in fact, a lawyer with that name and registration number who was licensed in the year You have specified.
c. If the information and/or evidence You provide does not correspond to the information on the records of the applicable jurisdiction’s licensing agency or bar association, We will send an e-mail to the e-mail address or call the telephone number listed on the jurisdiction’s records for this lawyer. If this lawyer has no e-mail address or telephone number, We will try to contact the attorney by mail. We will advise the lawyer in our communications that a person has applied for registration of an account on the Site using that lawyer’s credentials. Only after We take these steps and You cure any incorrect information will We show Your profile on the Site.
d. If in the registration process CloudLaw finds publicly available information that You have been or are the subject of a disciplinary action by the licensing board in the jurisdiction in which You purport to have a license to practice law, CloudLaw may make that information publicly available on Your profile. You consent to our doing that.
e. Subsequent to Your initial registration, CloudLaw may undertake periodic random updates of the status of Your license(s) to practice law in each jurisdiction(s) in which You purport to have a license to practice law and post the results of that update in Your profile. You acknowledge that We have entered into a Licensing Agreement with the State Bar of Georgia under which information will be provided to the Site by the State Bar of Georgia. If this information includes evidence of Your license to practice law in Georgia We will rely on such information and paragraphs 12 (a), (b) and (c) shall be inapplicable to You.

Coverage Attorney hereby represents and warrants to MMC and to the Clients that the specifics of the resume and other employment, education and bar admission information as provided by Coverage Attorney to MMC is complete and accurate in all respects and constitutes a material portion of the inducement to MMC to enter into this Agreement and will be a material element of the inducement to Clients to agree to engage the services of Coverage Attorney. Coverage Attorney agrees to indemnify and hold MMC harmless from all claims alleged against or damages suffered by MMC which are attributable in part or in whole to any omissions or inaccuracies as contained in any such information. Coverage Attorney represents and warrants that he or she is eligible to practice law and is in good standing with the respective state Bar in which he or she intends to practice. In the event Coverage Attorney is sanctioned, reprimanded, or in any way disciplined by any state Bar association or becomes ineligible to practice law for any reason he or she agrees to immediately notify MMC and not to accept or perform any additional work for MMC or MMC’s clients
Verification of Credentials. Certifications (e.g., RID, NAD, VQAS) are verified to ensure they are valid and current. • Licensing requirements specific to state or federal guidelines are confirmed.

Except as set forth in Section 3.35 of the PSR Disclosure Schedule, PSR Recruiting as general partner and the Partnership have implemented policies and procedures to verify the credentials (including, but not limited to, with respect to education and licensure) of personnel that the Partnership places with its clients and to collect, maintain and update such credentialing information. To the Knowledge of PSR, employees and consultants of PSR Recruiting and the Partnership each consistently follow and have followed such policies and procedures.
