Common use of Documentation of Services Clause in Contracts

Documentation of Services. Contractor shall provide all pertinent documentation required for state and federal reimbursement including but not limited to Consent Forms, assessments, treatment plans, and progress notes. Contractor agencies must submit, via fax to Quality Management at ▇▇▇-▇▇▇-▇▇▇▇, their version of these forms for approval before the forms are to be used. Special attention must be paid to documentation requirements for residential treatment facilities.
