Documentation of Services. (a) Contractor shall maintain documentation of services provided that includes each trip, the Member ID, the destination, the reason the ride was requested (service reason), and any incidents of no-show on part of the driver or the Member. (b) Contractor shall pay for coordination and provision of NEMT provided for Members if the Member is eligible for NEMT.
