Dues Check. Off During the term of this Agreement, the Company agrees to deduct monthly, an amount equal to the dues as levied by the Union for each pay period (weekly, or semi-monthly, etc.). The amount of regular Union dues to be deducted shall be furnished to the Employer by the Union. The deducted dues shall be remitted to the Union no later than the fifteenth (15th) day of each month following the month in which the deductions are made with a statement showing the names of the employees from whom deductions have been made and the amount deducted. The amount of dues to be deducted may be amended by the Union upon giving the Company fourteen (14)-calendar day’s written notice. The new deductions will take effect on the payday in the next week following the expiry of such notice. The Company shall provide the Union with a monthly computer disk detailing the following information: Employee name, address and employment status; Gender; Classification, salary and date of hire; The amount of gross dues deducted for each employee, any employee who has left or joined the Company since the last payment, including the name of any employee going on or returning from child care leave or any type of leave of absence as defined by this collective agreement. Dues Check. Off The Union may secure authorization for payroll deduction for Union dues. Such authorization may be revocable as provided by law. The Committee will request the Treasurer of the City of Amesbury to submit such sums to the Union Treasurer no later than ten (10) days after such deduction is made. Dues deduction will be standard monthly for all members.

3.01 The City agrees to deduct once each pay period, the Union Dues of said employees who individually and voluntarily certify in writing on the prescribed Due Permit form (Appendix A), that they authorize such deductions. No authorization shall be allowed for payment of initiation fee, assessment, political party or political action contributions or fines. The Secretary of the Union will notify the City of any subsequent change in the amount of the Dues to be deducted, and will so certify. Any change in the amount of the Union's dues would be effective in a time reasonable to allow the City to make the necessary technical and administrative payroll changes and program adjustments.
3.02 The City will deduct a monthly amount of $5.00 to cover administrative costs associated with Union dues' collection and remittance to the Union Treasurer.
3.03 The Union will indemnify, defend, and hold the City harmless against any claims made and against any suit instituted against the City on account of check-off of Union dues.
3.04 Any employee may withdraw his membership in the Union upon written request and thirty (30) days' notice to the City and Union. APPENDIX A -Authorization Form for Payroll Deduction of Dues: By: To: Effective: I hereby request and authorize you to deduct from my earnings once each month, an amount established by the Union as dues. The amount deducted shall be paid to the Treasurer of the Union. Off The Employer shall deduct from the pay of each employee covered by this Agreement, an hourly local working assessment of .0275% (rounded to the nearest nickel) of their total gross package for each hour paid. Dues Check. OFF FORM
1. When a member of the APFA properly executes such "Check-off Form", the APFA National President shall forward an original copy to American Airlines – Payroll Customer Service.
2. Any Check-off Form which is incomplete or improperly executed will be returned to the President.
3. Any notice of revocation, as provided for in Section 35 or in the Railway Labor Act, as amended, must be in writing, signed by the Flight Attendant and delivered by certified mail, addressed to the Manager, Payroll Customer Service, at the address indicated above, with a copy to the APFA National President.
4. Check-off Forms and notices received by the Manager, Payroll Customer Service, will be stamp-dated on the date received and will constitute notice to the Company on the date received, and not when mailed.

1. The Company will deduct from the pay of each Flight Attendant covered by this Agreement an amount equal to the standard monthly membership dues, service charges, initiation fees and assessments, uniformly levied in accordance with the Railway Labor Act, as amended, and the Constitution and By-laws of the Union, provided such Flight Attendant Union executes the following agreed upon form known as a “Check-Off Form.”
2. For Flight Attendants who have executed a Check-Off Form, deductions will be made in the following manner:
a. One-half of the deduction for dues or service charge will be made in the first paycheck of the month, and one-half of the deduction will be made in the second paycheck of the month.
b. Initiation fees will be split into equal payments each paycheck over the period of six (6) months. I, , hereby authorize and direct FRONTIER AIRLINES to deduct from my pay dues, in an amount equal to such monthly dues, service charges, initiation fees, and/or assessments, as now or may hereinafter be established in accordance with the Constitution and By-laws of the Union, for remittance to the Association of Flight Attendants-CWA. I agree this authorization will be irrevocable for one (1) year from the date hereof or until termination of the Check-Off agreement between Frontier Airlines and the Association of Flight Attendants-CWA, whichever occurs sooner. If the Check-Off agreement is terminated this authorization will be automatically terminated. In the absence of a termination of the Check-Off agreement, this authorization may be revoked effective as of any anniversary date of the signing hereof by written notice given by me to Frontier and the Association of Flight Attendants-CWA by certified mail, return receipt requested, during the ten (10) days immediately preceding such anniversary.
3. During Flight Attendant’s initial training, the Company will make known the dues Check-Off provisions of this Agreement. Those desiring to participate in the payroll deduction plan for the remittance of dues will at the time complete a copy of the above Check-Off form.
a. A copy of the completed form will be forwarded to the Union’s headquarters office and to the Company’s Payroll Department. In accordance with the Union’s Constitution and By-laws, the Payroll Department will begin the appropriate deduction.

Dues Check. OFF The Board agrees to deduct, once each pay period, dues and assessments in an amount certified to be current by the Secretary-Treasurer of the Union from the pay of those employees who authorize in writing that such deductions be made. All employees shall file authorization forms with The Board, signed by each employee prior to such deduction. The Board shall forward the sum total of all dues deductions to the Secretary-Treasurer of the Union no more than five (5) business days after each payday. If, during the life of this Agreement, there will be any change in the rate of the Employee's dues, the Union will furnish the Board written notice thirty (30) days prior to the effective date of such change. Any employee who is not a member of the Union shall, as a condition of employment, pay a bi-weekly service charge equivalent to eighty-five percent (85 %) of the dues and assessments paid by a member of the Union. The purpose of this is to offset the employee's cost of services rendered by the Union as a majority representative. Pursuant to the Agency Shop Law, any new employee in the bargaining unit on the effective day of this Agreement who does not join the Union within thirty days (30) of initial employment within the bargaining unit and any employee previously employed within the unit who does not join within ten (10) days of reentry into employment with the unit shall as a condition of employment pay a representation fee to the Union by automatic payroll deduction. Employees who fail to meet this requirement shall be disengaged. The Union will provide to the Board, a copy of its demand and return system and the Union shall indemnify, defend, and save the Board harmless against any and all claims, demands, suits or other forms of liability that shall arise out of, or by reason of action taken by the Board in reliance upon salary deduction authorization cards or the bi-weekly service charge information as furnished by the Union to the Board, or in reliance upon the official notification on the letterhead of the Union and signed by the President of the Union, advising of such changed deductions.
Dues Check off A Member is not required to join the Association as a condition of employment. However, all Members, whether or not they join the Association, shall pay the equivalent of the Association dues.
