Employee to Inform. ‌ (a) The employee shall advise the supervisor or designated person in charge a minimum of two hours prior to an 0700 scheduled shift and a minimum of four hours prior to all other scheduled shifts of her/his inability to report to work because of illness or injury, the nature of the illness or injury, and the probable date of her/his return to work. (b) Employees who are absent from work because of sickness shall contact their supervisor or the designated person in charge on a regular basis regarding the status of their condition and/or the anticipated date of return to work. (c) Employees who have been absent from work due to illness or injury must provide sufficient notice to the Employer prior to their return to work. (d) The Manager of Care or designate or the Director of Care or designate may require a Certificate of Fitness (Appendix 5) prior to the employee resuming work.

Employee to Inform. ‌ (a) The employee shall inform their immediate supervisor as soon as possible in advance of the scheduled shift, of an inability to report to work because of personal illness or injury. Employees shall make every reasonable effort to notify their supervisor at least four hours prior to the start of their shift in order to provide enough time to find replacement staff. (b) When an employee has not provided a doctor's note or certificate of medical absence, as requested by the Employer, within three working days of return to work, they will be considered to have been ineligible for sick leave.

Such Purchaser acknowledges and agrees that neither the Placement Agent nor any Affiliate of the Placement Agent has provided such Purchaser with any information or advice with respect to the Securities nor is such information or advice necessary or desired. Neither the Placement Agent nor any Affiliate has made or makes any representation as to the Company or the quality of the Securities and the Placement Agent and any Affiliate may have acquired non-public information with respect to the Company which such Purchaser agrees need not be provided to it. In connection with the issuance of the Securities to such Purchaser, neither the Placement Agent nor any of its Affiliates has acted as a financial advisor or fiduciary to such Purchaser.

  • Duty to Inform Licensee further warrants that it understands the imminent dangers (INCLUDING SERIOUS BODILY INJURY OR DEATH FROM ELECTROCUTION) inherent in the work necessary to make installations on the District’s Poles by Licensee’s employees, agents, contractors or subcontractors, and accepts as its duty and sole responsibility to notify and inform Licensee’s employees, agents, contractors or subcontractors of such dangers, and to keep them informed regarding same.

  • Right to Information The City of ▇▇▇▇▇▇ reserves the right to use any and all information presented in any response to this contract, whether amended or not, except as prohibited by law. Selection of rejection of the submittal does not affect this right.
