Evaluation and Reporting. 1. Grantee shall collect and submit to HHSC or its designated evaluation contractor and/or SAMHSA the NOMS measures by required timeframes. a. PIPBHC specific health outcomes must be collected at baseline, at six-month intervals post baseline, and at discharge. 2. Grantees will collect and report data needed for the SAMHSA Infrastructure, Prevention, and Promotion (IPP) performance measures as listed in the PIPBHC grant application. 3. In addition to PIPBHC measures, grantees will also report on other outcome measures and policies that have been incorporated into operating procedures. 4. Grantee will host internal team coordination meetings monthly to consistently consider issues or barriers to the provision of high-quality services, determine and implement solutions, and monitor results. Formal reviews of data will be conducted on a quarterly basis and reported to HHSC. 5. Grantee shall collaborate with HHSC and/or a third party contractor to measure outcomes of local efforts and contribute to project reports. 6. Grantee shall submit quarterly progress/status reports to the HHSC Project Director or designee to include a summary of activities related to and the progress made toward the completion of the deliverables set forth in this contract, as well as the status, barriers, and recommendations related to the accomplishment of these deliverables. A copy of any supporting materials for assessment activities will also be submitted. 7. Grantee shall use electronic health record (EHR) population management tools in order to support continuous quality improvement processes and must regularly generate reports by specific conditions to use for quality improvement, reduction of disparities, research, or outreach.
