Extended Health Clause Samples
The Extended Health clause defines the provision of additional health benefits beyond standard medical coverage for employees or members. Typically, this clause outlines coverage for services such as prescription drugs, vision care, paramedical services (like physiotherapy or chiropractic care), and sometimes emergency travel medical insurance. By specifying the scope and limits of these extra benefits, the clause ensures that individuals have access to a broader range of healthcare services, addressing gaps not covered by basic health plans and providing greater financial protection against health-related expenses.
Extended Health. Fifty percent (50%) of the billed premium towards coverage of eligible nurses in the active employ for the Extended Health Care Benefits as provided under the VON National Group Insurance Plan, provided that the balance of the premium is paid by each nurse through payroll deductions.
Extended Health. The Colleges agree during the term of this Agreement, to contribute one hundred per cent (100%) of the present premiums towards the current Extended Health Benefit Plan subject to the eligibility requirements provided under such Plan.
Extended Health. 7.1.3.1 Default level is: • $25 annual deductible single, couple or family • 100% reimbursement of eligible expenses including semi-private hospital room, • paramedical services at $750 per practitioner per person per year, • private duty nursing at $15,000 per year (after age 65 the lifetime maximum becomes $25,000), • hearing aid maximum at $1,000 in any four consecutive years, • international travel assistance for out-of-Canada coverage, • prescription drugs, and • other costs prescribed by a medical practitioner.
Extended Health. Group Life Coverage
Extended Health. Any teacher whose personal illness extends beyond the period compensated may be granted a leave of absence, upon proper application. Any leave granted shall be without pay. After an extended personal illness, the District may request that the teacher present a statement from the physician certifying the teacher's fitness for return to work.
Extended Health i. The Board will contribute one hundred per cent (100%) of the premium of the Extended Health Benefits Plan as established by Pacific Blue Cross or its successor.
ii. A monthly charge of fifty cents ($0.50) for single coverage and one dollar fifty cents ($1.50) for family coverage shall be added to the costs for Medical Services and shared equally between the employee and the Board to provide additional prosthetic equipment and other items such as heavy-duty wheelchairs, etc. when recommended by a physician.
iii. The Extended Health Benefits Plan shall include the following:
Extended Health. Fifty percent (50%) of the billed premium toward coverage of eligible employees in the active employ of the Employer for an Extended Health Care plan as provided under the VON Canada Flex Group Benefits Plan.
Extended Health. The Board shall pay seventy-five percentum (75%) and the employees twenty-five percentum (25%) of the cost of the Extended Health Benefits Plan. The Plan's Vision Care coverage will be $150 per family member per twenty-four
Extended Health. The Board shall pay fifty (50%) of the premium cost of the Provincial Extended Health Benefit Plan for each full-time and part-time teacher employed by the Board. The plan will currently include the following:
i. supplemental travel rider payable at one hundred percent (100%) reimbursement for the following Medical Travel Expenses:
(1) Transportation for a member and/or dependents by scheduled air, rail, ferry or bus to and from the nearest locale equipped to provide the treatment required, when ordered by the attending physician and surgeon because, in their opinion, adequate treatment is not available locally. Said transportation must be within two (2) months of referral and will not be provided to points beyond Vancouver, B.C.
(2) Transportation of an attendant for the patient being transported under B.11.3.b.iii.1 above, when ordered by the attending physician and surgeon.
(3) Accommodation in a commercial facility for the patient only, before and after medical treatment to a maximum of $45/day for a total of seven days. Accommodation is not provided for the attendant.
Extended Health. Following ninety (90) calendar days of service with the company, all employees, including their families, are entitled to benefits under the provisions of the insurance company’s Health Care Plan, with an annual deductible of twenty-five (25) dollars per employee for Single coverage and fifty (50) dollars for Family coverage for all covered expenses except prescription drugs. The prescription drugs portion of the Extended Health Plan will be covered by a generic drug plan card (two (2) cards to be issued per family), provided by the insurance company. Each purchase of prescription drugs under this plan will be subject to a five (5) dollar deductible amount to be paid by the employee at the time of each purchase.
