First Level of Review. The Association and/or the employee shall present the formal grievance in writing to their supervisor within ten (10) working days of completion of the informal process. The written grievance shall contain the following information: a. Name of employee and job title; b. Department/Section; c. Clear and concise statement of the nature of the grievance including the circumstances and dates involved; d. The specific provision(s) of the MOU or Personnel Rules alleged to have been violated; e. Requested remedy; f. Name of the employee's representative, if any; g. Date and signature of the employee and/or Association representative. The Supervisor shall render a decision and provide comments in writing and return them to the employee within ten (10) working days after receiving the written grievance. If the employee does not agree with their Supervisor's decision or if no answer has been received within the ten (10) working days, the employee may present the grievance in writing to the Department Head or designee.
