Formal Evaluation Procedures. The appropriate supervisor shall evaluate each employee formally in writing. The following procedures shall be used: A. Conference and Copy of Evaluation A copy of each formal evaluation shall be given to the employee and a conference shall be held between the employee and the immediate supervisor. Only evaluation documents that have been signed and dated by both the employee and the appropriate supervisor shall be contained in the employee's evaluation file. No employee shall be required to sign a blank or incomplete evaluation form. Employees shall, however, sign all completed evaluation documents. B. Responses If the employee feels that his/her formal evaluation is incomplete, inaccurate, or unjust, he/she shall have fourteen (14) days to put his/her objections in writing and have such attached to the evaluation report to be placed in his/her evaluation file.
