Formal Level I. If, as a result of the informal discussion with the immediate supervisor the complaint is not resolved, a formalized grievance shall be submitted in writing to the Human Resources office within fifteen (15) days following the date of the alleged grievance. The written grievance to the Human Resources office shall include: a. Identification of the grievant(s). b. The specific facts upon which the grievance is based. c. Identification of the applicable portion of the Agreement allegedly violated, misinterpreted, or misapplied. d. The specific relief requested. e. The date on which the alleged grievance occurred. f. The date on which the grievance is being filed. g. A signature attesting to the facts as presented.
