Grievance Levels. 1. If an issue is not resolved by informal discussions with the supervisor, within the timeline cited in Article 6.1.3, the grievance will be submitted in writing on the negotiated grievance form. 2. The grievance must include the following information: a) The date of the alleged incident b) The nature of the grievance c) The facts upon which it is based d) The specific article and section of the Agreement violated e) The specific remedy requested f) The name of the grievant g) The name and signature of the authorized Union representative 3. The Union will file the grievance on behalf of the member(s) at the appropriate level. a) LEVEL ONE – Immediate Supervisor b) LEVEL TWO – Second Line Supervisor or Designee
