Hazard Identification. (a) The safety and health representative for all offices and supervisor (at least one [1] of each) evaluates all office equipment, work areas and work procedures for every job in each workplace to identify potential sources of harm to workers. This must be done at least once annually (fiscal year end March 31) and whenever there is a change in office equipment, work areas or work procedures. (b) The safety and health representative and supervisor will identify, record and prioritize each hazard using hazard identification worksheets. This information will then be forwarded to management for action and/or approval. One (1) copy is filed at the workplace and another is submitted to the Safety & Health Committee.
