Immediate Notification. If your Card or PIN is lost or stolen, or if you believe either your Card, PIN, or the account number for your Card is subject to any unauthorized use, you should notify us immediately, and take any reasonable actions we may request to prevent unauthorized Card use. Contact us immediately if you believe: (a) a Card has been lost or stolen, (b) someone has gained unauthorized access to any access information for your Account, or (c) someone has transferred or may transfer funds from the Card without your permission.

Immediate Notification. Vendor’s ordering system will provide notice within twenty-four (24) hours to the MMCAP and its Members of any Products covered by this Agreement that the Vendor has placed on backorder. Vendor’s backorder notification will include: i. the Products placed on backorder status; ii. the expected timeline of the backorder; iii. the reason for the stock outage was caused; and iv. how the Vendor intends to resolve the backorder situation.

Immediate Notification. Cardinal Health and Cumberland agree to notify the other party as soon as reasonably practicable of any information that each may obtain or learn concerning any Product or package complaint or any serious unexpected side effect, injury, toxicity, or sensitivity reaction or any unexpected incidence of severity thereof associated with the clinical uses, studies, investigations, tests and marketing of the Products, whether or not determined to be attributable to the Products. “Serious” as used in this Section 11.1 refers to an experience which results in death, permanent or substantial disability, in-patient hospitalization, prolongation of existing in-patient hospitalization, a congenital anomaly or cancer, or a result of an overdose or life threatening condition. “Unexpected” as used in this Section 11.1 refers to (i) conditions or developments not previously submitted to governmental Agencies or encountered during clinical studies of the Products and not reflected in the Product Promotional Materials or the Product Labeling, or (ii) conditions or developments occurring with greater frequency, severity, or specificity than shown by information previously submitted to governmental Agencies or encountered during clinical studies of the Products and not reflected in the Product Promotional Materials or the Product Labeling. Each party shall also notify the other in a timely manner of any other adverse experience, i.e., any unfavorable and unintended change in the structure (signs), function (symptoms) or chemistry (laboratory data) of the body temporally associated with the use of the Products, whether or not considered related thereto.

Immediate Notification. The City will notify the State District Engineer's authorized representative immediately upon the contractor encountering contaminated soils and/or groundwater in areas that are within the Project limits. The City will confer with the State as to the handling, disposal, and any other issues related to contaminated materials found on State Right-of-Way or import of materials onto State Right-of-Way.

Therefore, all RHA members shall immediately contact the RHA Board and provide all pertinent information regarding any allegation of discrimination, abuse or potential abuse, including ethnic, racial, sexual, physical or mental abuse, or consumption, use, sale, gift or abuse of mood altering substances.

(7) Proper precautions to take as a result of the release, including evacu- ation (unless such information is read- ily available to the community emer- gency coordinator pursuant to the emergency plan). (8) The name and telephone number of the individual (or individuals) to be contacted for further information.

Immediate Notification. Vendor or Vendor’s ordering system will provide notice within twenty-four (24) hours to MMCAP Infuse and its Members of any Products covered by this Agreement that the Vendor has placed on backorder. Once the Product becomes available, orders are released and fulfilled automatically unless the Member advises otherwise. Vendor’s backorder notification will include: i. Members ordering directly will be notified at the time of ordering if a particular product is not available and will be given the option to place the order for the product that will remain in back order status; ii. For extended back orders, support will be provided to find an appropriate course of action; and iii. Member can contact Vendor’s Customer Service to obtain details regarding the order and shipping ETA of when the product will become available.
