Levels of Accomplishment. The services described in Exhibit A shall be delivered at a level which is reasonably comparable to the service level projected on the 2023 Agency Request for Funds Form. If the level of services is estimated to be appreciably different from that noted on the 2023 Agency Request for Funds Form, the difference shall be justified to the satisfaction of LHA.

Levels of Accomplishment. In addition to the normal administrative services required as part of this Agreement, the Subrecipient agrees to provide the levels of Services as more fully described in Exhibit “A”.
Levels of Accomplishment. Senior Care Volunteer Network shall log every trip and register all riders. A trip is defined as a one way ride when a driver is transporting a client. Senior Care Volunteer Network shall provide a monthly report to the County which includes rider, trip and mileage data. A representative from Senior Care Volunteer Network shall attend quarterly Public Transportation Advisory Committee (PTAC) meetings. Updates on ridership figures, program updates, and/or pertinent information shall be presented at these PTAC meetings at the request of MCDOT staff.

Performance Monitoring The Grantee will monitor the performance of the Subrecipient against levels of accomplishments required herein. Substandard performance as determined by the Grantee will constitute non-compliance with this Agreement. If action to correct such substandard performance is not taken by the Subrecipient within a reasonable period of time after being notified by the Grantee, Agreement suspension or termination procedures will be initiated. The Subrecipient shall assist the Grantee in measuring program impact to determine the extent to which the activities yield the desired outcomes in the community or in the lives or persons assisted. The Subrecipient shall assist the Grantee in selecting indicators that relate to the local goals established in the Consolidated Plan.
