In developing the plan consideration shall be given to retraining, mentoring and/or additional supervision requirements. The action plan should take account of factors that can have caused the sub- standard performance such as: • changes in ability or loss of knowledge; • lack of opportunity to practise the competence; • lack of confidence or experience; • lack of willingness; • changes in shift patterns; • lack of concentration; • poor team working; • personal reasons; • poor relationships at work; • change in health and fitness; • omissions or deficiencies in a previous assessment.
