Common use of Measurable Outcomes Clause in Contracts

Measurable Outcomes. 1. To be submitted in accordance with the Work Plan, reporting on domains selected on the funding application. 2. Pursuant to the Work Plan, unless otherwise authorized by the Mental Health Board, all funded programs will adopt measurable outcomes. Outcome reports are required to be completed using mechanisms and timeframes as set forth by the Mental Health Board. Outcome reports shall be completed and submitted to the Mental Health Board in the specified format by the last day of the month following the end date of each reporting period. 3. Provider is required to maintain the use of the assessment tool(s) identified on the Work Plan to measure outcome domains. 4. In order to ensure accuracy and continuity for outcomes reporting and persons served each quarter, Provider must make available at time of compliance audit, written documentation of the process or procedure by which the Provider measures, accumulates, calculates and reports applicable data, including unduplicated persons served as required per Work Plan.
