Common use of Notification of Change of Status Clause in Contracts

Notification of Change of Status. 25.01 Every employee shall be individually responsible for notifying the Hospital within five (5) working days of change of address, phone number, marital status, name, income tax status, insurance beneficiary, next of kin, and any other reasonable information pertaining to personnel records.
