Performance Measure Target. Senior households placed in housing from County-approved shelters with housing subsidies through 6/30/28 • 10-12 senior households, representing 10-24 persons Conduct case management meetings to: o Assist clients with housing placements and scholarship allocation. o Connect clients to community resources (i.e. PG&E, food, employment, medical, etc.). o Explore future housing scenarios, including applying to deed- restricted affordable housing. o Assist clients with getting on the MTW Voucher waiting list. Should a senior be selected for an alternative housing option (i.e., MTW voucher), they will transition from the housing subsidy, allowing us to apply the remainder to a new shelter client. o Connect clients to Balance for financial coaching and debt reduction activities. • Monthly meetings for 10-12 of senior clients Conduct presentations to shelter residents to increase interest in Consultant’s programs, increase referral numbers and increase the number of shelter families being housing through the Self- Sufficiency Program (SSP) and to achieve the following goals: o Share information about the SSP Program with shelter staff and residents through presentations and one-on-one meetings. o Onboard new shelter staff about the SSP Program and referral process. o Identify families who haven’t yet been referred to the SSP Program but could be. o Create and share a monthly flyer about SSP’s SHARE vacancy and housing subsidy availabilities with shelter staff and residents. o Work with shelter staff to coordinate job fairs and other career development resources for residents. o Administer a career pre-assessment to help parents understand the possibilities for their educational and career goals. o Coach families on opportunities to enroll in career and educational programs using partners such as VRS, Job Train, NOVA works, local community colleges, etc. o Provide one-on-one mentoring with families to explore career and educational paths as they relate to the SSP Program. • 25-30 presentations at a San Mateo County shelter o Provide one-on-one mentoring to explore the opportunities available through the SSP Program. o Invite fathers to the SSP’s Nurturing Fathers program. Cultivate and follow up with prospective SSP families as part of the pre-program entry process. • 5-6 families (10-18 people depending on household size) Select families to enter the SSP program and receive housing subsidy and placements. • 2-3 families (4-9 people depending on household size)

  • Performance Measures The System Agency will monitor the Grantee’s performance of the requirements in Attachment A and compliance with the Contract’s terms and conditions.

  • Performance Measurement The Uniform Guidance requires completion of OMB-approved standard information collection forms (the PPR). The form focuses on outcomes, as related to the Federal Award Performance Goals that awarding Federal agencies are required to detail in the Awards.

  • Ongoing Performance Measures The Department intends to use performance-reporting tools in order to measure the performance of Contractor(s). Such measures will allow the Department to better track Vendor performance through the term of the Contract(s) and ensure that Contractor(s) consistently provide quality services to the State and its Customers. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MFMP or on the Department's website).
