Performance Outcome Objectives. 22 a. Objective 1: CONTRACTOR shall provide effective residential substance abuse 23 assessment, treatment, and counseling to Clients with identified alcohol and/or drug problems as 24 measured by Retention and Completion Rates:
25 1) Retention Rates shall be calculated by using the number of Clients currently 26 enrolled in or successfully completing the treatment program divided by the total number of Clients 27 served during the evaluation period.
28 2) Completion Rates shall be calculated by using the number of Clients successfully 29 completing the treatment program divided by the total number of Clients discharged during the 30 evaluation period. Fifty percent (50%) of Clients will complete residential treatment program.
31 b. Objective 2: CEST scores at midpoint and completion will be higher than national 32 norms in perceived social support, peer support, counseling rapport, and treatment participation. CEST 33 scores for treatment readiness and desire for help will exceed national norms and will be equal to or 34 higher than CESI scores at intake;
35 c. Objective 3: Ninety percent (90%) of Clients who complete satisfaction survey will 36 agree or strongly agree that they are “overall satisfied with the services received” and ninety percent 37 (90%) or Clients surveyed will agree or strongly agree that they would recommend the program to 1 someone they know; and

7 2) Reduce no-shows
8 3) Increase admissions
9 4) Increase continuation in treatment

16 b. Objective 2: CONTRACTOR shall have the Participant complete the CESI for eighty 17 percent (80%) of Participants at time of intake. The CEST shall be completed at mid-point and at 18 completion for those Participants receiving, at a minimum, forty-five (45) calendar days of treatment.
19 1) CONTRACTOR shall ensure that surveys are completed timely and accurately by 20 designated Participants. This would include, but is not limited to, ensuring surveys contain provider 21 number, Participant ID number, responses to all psychosocial questions, along with other important 22 Participant and CONTRACTOR information, and fields are filled and/or marked appropriately.
23 2) CONTRACTOR shall photocopy the CESI and CEST surveys and submit the 24 originals to ADMINISTRATOR for the COUNTY, once a month, by the tenth (10th) business day of 25 each month.
26 3) CONTRACTOR shall maintain photocopies of the CESI and CEST documents in 27 Participant files.
28 4) CONTRACTOR shall adhere to all COUNTY CESI and CEST transmission, 29 reporting, sorting, and any other guidelines, as stipulated by ADMINISTRATOR, as they may now exist 30 or as they may be revised and/or amended in the future, for the review, use and analysis of the CESI and

Performance Outcome Objectives. 5 A minimum of eighty percent (80%) of Clients served will be successful in finding 6 permanent housing. 7 A minimum of eighty percent (80%) of Clients successfully placed in permanent 8 housing will remain housed after one (1) year from the date of original intervention.

6 b. Objective 2: Completion Rates shall be calculated by using the number of Clients who 7 leave with satisfactory progress divided by the total number of Clients discharged during the evaluation 8 period. Seventy percent (70%) of Clients will complete residential treatment program.
9 c. Objective 3: Provide linkage to the next level of care for Clients upon discharge. Thirty 10 percenty (30%) percent of Clients who have discharged will be linked with a lower level of care within 11 thirty (30) calendar days, as measured by charge data entered into the IRIS. Linkage rates for Clients who 12 discharge will include all CalOMS standard discharge dispositions. All CalOMS administrative discharge 13 dispositions will be excluded.

Linkage rates for 26 Clients who discharge will include all CalOMS standard discharge dispositions. All CalOMS 27 administrative discharge dispositions will be excluded.
