Procedures for Reimbursement. When an employee experiences a loss that meets the conditions described in the Agreement on Reimbursement for Personal Property Damages, he/she should follow these procedures. 1. Inform his/her administrative supervisor of the loss and the circumstances surrounding it. This should be done within one week of the date of the loss. 2. Write a request for reimbursement to the Associate Superintendent of business services fully describing the loss and circumstances. Have the administrative supervisor sign the request indicating that he/she has reviewed the matter and has found the description to be accurately stated. This request should be made within two weeks of the incident. 3. Provide the Associate Superintendent of business services with a receipt of replacement or repair costs and, if appropriate, a copy of an insurance document or letter showing the amount of coverage and deductible. This material should be furnished within one month of the date of the incident. 4. If the Associate Superintendent of business services finds that the request and supportive documentation is in conformity with the provisions of the agreement, then the employee will be reimbursed.
