Projected Enrollment. For the purpose of funding students enrolled in the Charter School each year the Charter School offers a new grade level, the Charter School may be required to provide the SCSC a projected student enrollment count that includes prospective student names, Georgia Testing Identifier (GTID), if available, and any other information as requested by the SCSC. The Charter School shall provide this information by the deadline established by the SCSC and in the form and manner as requested by the SCSC. The information provided by the Charter School pursuant to this section may be verified by the SCSC through an onsite visit or by other means.

Projected Enrollment. 1) Projected enrollment for the following school year on or before December 1 (for District planning purposes). 2) Projected enrollment for the following school year on or before March 15 per Section 7.4 (for funding purposes).
Projected Enrollment. Per the Agreement with Philadelphia University and Reliance Medical Group, The maximal number of students admitted into the Philadelphia University MS in PA Studies NJ Program entering from the Combined 5-Year BSHS/MS will be no more than 50% of the entering graduate class in any given year. Entering class size is determined by Philadelphia University with the approval Accreditation Review Commission on Education for the Physician Assistant (ARC-PA). It is anticipated that Philadelphia University will receive approval to offer the MSPA in New Jersey during the 2012-2013 or 2013-2014 academic year. Therefore, the first group of freshman would likely be admitted to the BSHS-MSPA Fall 2014. Philadelphia University enrollment projections and BSHS-PA enrollments are as described in Table 2. Projected Enrollment. By June 1, 2015, and by no later than May 1 for each subsequent year thereafter during the term of this Agreement, the Parties shall prepare a projection of the number of students reasonably likely to be enrolled in the School in the next school year (“Projected Enrollment”). The Parties shall determine the School’s Projected Enrollment in substantially the same manner as it is determined for other IPS schools.
Projected Enrollment. The proposed number of students to be enrolled must be stated and the method of determining potential enrollment must be identified.
Projected Enrollment. The estimate for the number of students in entering cohorts is a reflection of the interest in the program from the survey responses. In addition, recent history of enrollment into other Joint Degree programs at BCC with other units of CUNY shows that robust growth since their inception is expected due to strong student interest. For example, the Science for Forensic A.S/Forensic Science B.S. program between BCC and ▇▇▇▇ ▇▇▇ College has increased from 49 students at its initial offering to 101 in a space of three years. The Dietetics and Nutrition A.S/Dietetics, Foods, and Nutrition B.S. program between BCC and ▇▇▇▇▇▇ College has grown from 67 to 258 students in the five years since its inception. The Liberal Arts and Science, Enrollment estimates in the AS/BS program for the first five years are given below in Table 1. The numbers are calculated as follows: Entering cohorts will be of 16 full-time and 16 part-time students with about 40% of students transferring in with advanced standing. A large percentage of incoming freshmen will require remediation in at least one of the three areas of reading, writing and mathematics. This will increase the normal time of progress of full-time students from four to five or six semesters. BCC students will begin transferring to CCNY by year four. The attrition rate for each cohort is estimated to be 40% for the first year and 20% thereafter. These estimates are equal to college-wide statistics over the last three years at Bronx Community College.

  • Open Enrollment Period Open Enrollment is a period of time each year when you and your eligible dependents, if family coverage is offered, may enroll for healthcare coverage or make changes to your existing healthcare coverage. The effective date will be on the first day of your employer’s plan year. A Special Enrollment Period is a time outside the yearly Open Enrollment Period when you can sign up for health coverage. You and your eligible dependents may enroll for coverage through a Special Enrollment Period by providing required enrollment information within thirty (30) days of the following events: • you get married, the coverage effective is the first day of the month following your marriage. • you have a child born to the family, the coverage effective date is the date of birth. • you have a child placed for adoption with your family, the coverage effective date is the date of placement. Special note about enrolling your newborn child: You must notify your employer of the birth of a newborn child and pay the required premium within thirty -one (31) days of the date of birth. Otherwise, the newborn will not be covered beyond the thirty -one (31) day period. This plan does not cover services for a newborn child who remains hospitalized after thirty-one (31) days and has not been enrolled in this plan. If you are enrolled in an Individual Plan when your child is born, the coverage for thirty- one (31) days described above means your plan becomes a Family Plan for as long as your child is covered. Applicable Family Plan deductibles and maximum out-of-pocket expenses may apply. In addition, if you lose coverage from another plan, you may enroll or add your eligible dependents for coverage through a Special Enrollment Period by providing required enrollment information within thirty (30) days following the date you lost coverage. Coverage will begin on the first day of the month following the date your coverage under the other plan ended. In order to be eligible, the loss of coverage must be the result of: • legal separation or divorce; • death of the covered policy holder; • termination of employment or reduction in the number of hours of employment; • the covered policy holder becomes entitled to Medicare; • loss of dependent child status under the plan; • employer contributions to such coverage are being terminated; • COBRA benefits are exhausted; or • your employer is undergoing Chapter 11 proceedings. You are also eligible for a Special Enrollment Period if you and/or your eligible dependent lose eligibility for Medicaid or a Children’s Health Insurance Program (CHIP), or if you and/or your eligible dependent become eligible for premium assistance for Medicaid or a (CHIP). In order to enroll, you must provide required information within sixty (60) days following the change in eligibility. Coverage will begin on the first day of the month following our receipt of your application. In addition, you may be eligible for a Special Enrollment Period if you provide required information within thirty (30) days of one of the following events: • you or your dependent lose minimum essential coverage (unless that loss of coverage is due to non-payment of premium or your voluntary termination of coverage); • you adequately demonstrate to us that another health plan substantially violated a material provision of its contract with you; • you make a permanent move to Rhode Island: or • your enrollment or non-enrollment in a qualified health plan is unintentional, inadvertent, or erroneous and is the result of error, misrepresentation, or inaction by us or an agent of HSRI or the U.S. Department of Health and Human Services (HHS).

  • Open Enrollment KFHPWA will allow enrollment of Subscribers and Dependents who did not enroll when newly eligible as described above during a limited period of time specified by the Group and KFHPWA.

  • Enrollment You are responsible for i) having all of the required information in this Agreement completed and

  • Disenrollment 12.1 ADFMs shall be disenrolled from TOP Prime/TOP Prime Remote, TOP Select when: • The enrollee loses eligibility for TOP enrolled coverage, • The enrollee has not requested enrollment transfer/disenrollment of TOP Prime/TOP Prime Remote within 60 calendar days following the end of the overseas tour. 12.2 ADSMs shall be disenrolled from TOP Prime/TOP Prime Remote when: 12.3 ADFMs who are enrolled in TOP Prime/TOP Prime Remote may disenroll at any time. They will not be permitted to make another enrollment until after a 12-month period if they have already changed their enrollment status from enrolled to disenrolled twice during the enrollment year (October 1 to September 30) for any reason. ADFMs with sponsors E-1 through E-4 are exempt from these enrollment lock-out provisions. See Chapter 6, Section 1 for guidance regarding enrollment lock-outs. Effective January 1, 2018, see TPM, Chapter 10, Section 2.1 for QLE information and Chapter 6, Sections 1 and 2, for enrollment eligibility and time frames. 12.4 ADSMs cannot voluntarily disenroll from TOP Prime or TOP Prime Remote if they remain on permanent assignment in an overseas location where these programs are offered. ADSM enrollment in TOP Prime or TOP Prime Remote continues until they transfer enrollment to another TRICARE region/program or lose eligibility for TOP/TRICARE. 12.5 TOP Prime/TOP Prime Remote enrollees must either transfer enrollment or disenroll within 60 calendar days of the end of the overseas tour when the ADSM departs to a new area of assignment. The TOP contractor shall provide continuing coverage until (1) the enrollment has been transferred to the new location, (2) the enrollee disenrolls, or (3) when enrollment transfer or disenrollment has not been requested by the TOP Prime/TPR enrollee by the 60th day the TOP contractor will automatically disenroll the beneficiary on the 61st calendar day following the end date of the overseas tour from TOP Prime or TOP TPR. Until December 31, 2017, the disenrolled ADFM TOP Prime or TOP TPR beneficiary will revert to TRICARE Standard. Effective January 1, 2018, ADFMs disenrolled from TOP Prime or TOP TPR will be only eligible for space available care at military treatment facilities.‌
