Provider Data Clause Samples

The "Provider Data" clause defines the rules and responsibilities regarding data that is supplied or generated by the service provider during the course of a contract. Typically, this clause outlines what constitutes provider data, how it may be used, stored, or accessed, and any restrictions on its disclosure or transfer. For example, it may specify that the provider retains ownership of certain proprietary information or that the client must return or destroy provider data upon contract termination. Provider Data. As between Preventia and Provider, Provider exclusively owns all rights, to data or information submitted by Provider for view in the Application Services (“Provider Data”). As such, Provider agrees that it has entered into a Business Associate Agreement with Preventia and has the right to provide Provider Data to Preventia hereunder. In partial consideration for the Preventia Services provided herein, as well as other good and valuable consideration, Provider hereby grants Preventia a perpetual, royalty-free, irrevocable license to the Provider Data for Preventia to perform its obligation hereunder and an unlimited right and license to use the Provider Data on an aggregated, de-identified data in benchmark data sets and Provider hereby grants such authority to the extent Provider has the right to grant it to Preventia. Preventia may sell and sublicense the Provider Data for such Fees to its customers that utilize the Preventia Application.
Provider Data. Customer Offices acknowledge and agree that it is the Provider's responsibility, and not ▇▇▇▇▇▇▇' or the Amadeus ACOs', to ensure that the information supplied and stored in the Amadeus System is accurate and properly updated. Therefore, neither ▇▇▇▇▇▇▇ nor the Amadeus ACO will be liable for the content, accuracy, use or continued availability of any data or information contained in the Amadeus System.
Provider Data. It is the Provider’s responsibility to ensure that the data and Information supplied and stored in the systems provided hereunder is accurate and properly updated, and that neither Amadeus nor an Amadeus NMC has no responsibility or liability or obligation for the accuracy, content or reliability of such data or information, or EBOOKERS’s or any Website user’s access to or use of such data or information.
Provider Data. The contractor shall maintain individual and group provider network information with basic demographics, EIN or tax identification number, professional credentials, license and/or certification numbers and dates, sites, risk arrangements (i.e., individual and group risk pools), services provided, payment methodology and/or reimbursement schedules, group/individual provider relationships, facility linkages, number of grievances and/or complaints. For PCPs, the contractor shall maintain identification as traditional or safety net provider, specialties, enrollees with beginning and ending effective dates, capacity, emergency arrangements or contact, other limitations or restrictions, languages spoken, address, office hours, disability access. See Article 5. The contractor shall maintain provider history files and provide for easy data retrieval. The system should maintain audit trails of key updates. Providers should be identified with a unique number. The contractor shall be able to cross-reference its provider number with the provider's EIN or tax number, the provider's license number, UPIN, Medicaid provider number, and Medicare provider number where applicable.
Provider Data. Within three (3) business days after the Effective Date, Kaiser will deliver to the Company, directly or through any designated Plan TPA, a list of Plan Providers in the Territory (the "Provider List"). The Provider List shall set forth, to the extent available for each listed Plan Provider, (a) the name of the Provider group, (b) the number of physicians in the group, (c) the address, telephone number and estimated monthly volume of Claims Submissions of the group and (d) the names and specialties of the physicians in the group. During the Production Phase, if implemented, Kaiser will use its reasonable efforts to provide the Company with the foregoing information, to the extent available, for Plan Providers in additional regions of the Territory then included in the Program and to keep the Provider List current on a periodic basis.
Provider Data. As applicable, County and its Subcontractors will submit to CalOptima complete, accurate, reasonable, and timely provider data, Program Data, Template Data, and any other reports
Provider Data. Wayfair acknowledges that all NPI of, and all other information regarding, Wayfair Customers specific to the Services, including (without limitation) information related to a Request and information regarding the origination, servicing, and collections pertaining to the Services, as well as information relating to Customer interaction with any portal to service or otherwise administer leases (“Provider’s Portal”) located on Provider’s systems or websites (collectively, “Provider Data”), as between Wayfair and Provider, is owned by Provider.
