You confirm that you have fully and fairly disclosed the purpose for, and the nature of, the Event including but not limited to the number of proposed guests; the full name and address of the principal client (if any) for whom you are acting; the nature of your and the principal client’s (if any) business; and any entertainment, equipment or special effects which may constitute a special risk to the Venue.

Purpose of Event. I understand this Event is a community awareness, fundraising Event, designed to create community connections through music while raising funds to support the FREE awareness, education, and support programs of LiFE OF HOPE, dedicated to making suicide a talkable topic, reducing the rate of suicide ideation, attempt, and loss through peer-based programming. All profits from HOPE Notes Music Fest will be dedicated to the LiFE OF HOPE organization and those served by its mission. I hold no claim to any profits earned as a result of this Event now or in the future. Further, I forgo any payment of compensation for my promotion or participation in this Event, including future use of materials developed to promote LOH or future HOPE Notes Music Fest Events. Photographs, Name and Likeness: I hereby irrevocably permit, authorize, and license LOH and its affiliates to display, publicly perform, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, modify, alter, edit, adapt, exploit, and otherwise use and permit others to use my name, organization of employment, testimonials, biographical information, application entry, image, likeness, appearance, voice, and other personal characteristics and all materials created by or on behalf of LOH that incorporate any of the foregoing ("Materials") on a perpetual basis throughout the world and in any medium or format whatsoever now existing or hereafter created, without further consent from or royalty payment or other compensation to me. I hereby irrevocably grant to LOH a gratis use license for my entire right, title and interest, if any, in and to the Materials and all copyrights in the Materials arising in any jurisdiction throughout the world. I acknowledge and agree that I have no right to review or approve Materials before they are used by LOH, and that LOH has no liability to me for any editing or alteration of the Materials or for any distortion or other effects resulting from the LOH’s editing, alteration or use of the Materials. LOH has no obligation to use the Materials or to exercise any rights given by this Agreement. Purpose of Event. Licensee may use the clubhouse for personal events only, including but not limited to weddings, baby or wedding showers, engagement parties, graduation parties, birthday parties, and anniversaries. Licensee may not use the clubhouse for business, marketing, or fundraising events, nor may Licensee charge any fees for attendance or sell any goods or services from the clubhouse.
