Sessions. The first few sessions will involve an evaluation of your needs. During this time, we can both decide if I am the best person to provide these services in order to meet your treatment goals. By the end of this period of time, if I have not already done so, I will be able to offer you some impressions of what the work will include if you decide to continue with therapy. You should evaluate this information along with your own opinions of whether you feel comfortable working in this way. You always maintain the right to ask about other treatments for and about their risks and benefits. Once psychotherapy has begun, I usually schedule one 50-minute session (one appointment hour of 50 minutes duration) per week at a time we agree upon. Based on your needs, some sessions may be longer or more frequent. Missed sessions present a special problem in therapy because the given hour is specifically blocked out for you and often can not be filled on short notice. If you must miss an appointment, please notify me as soon as possible at (▇▇▇) ▇▇▇-▇▇▇▇. I will try to find another time to reschedule the appointment. Unless you give 48 hours notice of your intention to miss the session, you will be billed for the session. Insurance will not reimburse you for missed sessions. Sessions are 50 minutes long and are billed at the rate of $225 for individuals and $250 for couples and families. If this rate should change, you will be given at least 2 months notice. Longer or shorter sessions are pro-rated by prior mutual agreement. You will be expected to pay for each session at the time it is held unless we arrange for monthly ▇▇▇▇▇▇▇▇. If for any reason you should become involved in legal action that requires your therapist or other counseling clinical staff to protect your privilege of confidentiality or testify on your behalf, you must agree to cover legal expenses and the therapist's time. I do not agree to take on any cases where there is an established legal case or where you would expect me to testify in any such case. Our contract is for treatment purposes only. In the rare case of your records being subpoenaed by the courts, you will be expected to cover the cost incurred by me for the processing of the records.

You will receive the best benefit from treatment if you come to your appointments regularly. If you must cancel an appointment, this Agreement represents your promise that you will do so at least 24 hours in advance of your scheduled time. If you do not cancel 24 hours in advance, the BHCTC will need to charge you for a missed session because your clinician will have reserved that treatment time for you and will be unable to use that time to see any other patient. A missed appointment cannot be billed to your insurance carrier, which means that you will be responsible for paying the BHCTC in full a $100 cancellation fee for the missed appointment. The BHCTC recognizes that emergencies or other unplanned events do arise that cannot be helped (flat tires, sick children, family emergencies), so this Agreement allows you to cancel one (1) appointment less than 24 hours in advance within any 6 month period without any charge to you. If you cancel more than that, you understand and agree that BHCTC can bill you $100 for each missed appointment. PROFESSIONAL FEES. Fees vary depending on the services provided and the credentials of the counselor. A fee schedule is provided on the Professional Disclosure Statement. In addition to weekly appointments, this office charges for other professional services you may need. Other services may include report writing, telephone conversations lasting longer than 5 minutes, consulting with other professionals, preparation of records or treatment summaries, and the time spent performing any other service you may request. If you become involved in legal proceedings that require this office’s participation, you will be expected to pay for all professional time, including, but not limited to communication with any involved attorney; court testimony; and preparation; as well as any expenses or costs, such as transportation or parking. Due to difficulties associated with legal matters, this office charges a higher hourly rate for work related to legal matters. Please refer to the current fee schedule. Your counselor is often not immediately available by telephone. This may be because the counselor is seeing another patient or due to other work commitments. While your counselor will usually be in the office between 8:30 a.m. and 5:30 p.m., your counselor will not answer the phone when with other patients. Sessions. Once you have created an Account, you may initiate Sessions from your Account. You may also participate in a Session initiated by another Customer (e.g., as a Signatory or a Witness/Participant). If you are initiating a Session, you are solely responsible for determining whether the Transaction and any associated Documentation (i) may be signed electronically by you or other Signatories or Witnesses/Participants; (ii) if required, may be notarized using Remote Online Notarization; and (iii) that the recipient will proceed with the Transaction electronically. We do not represent and warrant, however, that any Notary using the Platform will agree to perform a Remote Online Notarization for any specific Transaction. For example, the Notary may determine that Remote Online Notarization is not allowed for such a Transaction. Further, certain laws applicable to a Transaction may impose requirements applicable to Transactions involving consumers. These requirements include, but are not limited to, that the consumer consent in the manner required by the applicable Electronic Signature Law. Provider is not responsible for (i) determining whether any Authorized Platform User is a consumer, (ii) providing any consent disclosures required by the applicable Electronic Signature Law, (iii) reviewing any consent disclosures provided by the Customer for use on the Platform for compliance with the applicable Electronic Signature Law, (iv) obtaining any consumer consent required by the applicable Electronic Signature Law, or (v) determining whether any Authorized Platform User has withdrawn any consent previously given. At the end of a Session, Provider will upload the notarized and/or electronically signed Documentation to Customer’s Account. Unless Customer has purchased Platform storage Services, Provider will only maintain such Documentation for a limited period and will notify Customer of that period. The Customer is solely responsible for ensuring that it has a copy of the Documentation.

A market data session is defined as a connection between the vendor and Omega which is intended for transmitting order and trade information via the ITCH 3.00 protocol for the purposes of, but not limited to, building a book, consolidating into a best bid and offer feed, routing, making trading decisions, and monitoring or compliance.
