Shift to Long Form. Should an evaluator determine that a teacher on a short form evaluation 2 should be moved to a long form evaluation for that school year, the teacher must be informed of this 3 decision in writing at any time on or before December 15.
Shift to Long Form. In the event an evaluator gives written notice to a short form eligible employee prior to the end of the first (1st) semester, of any school year, that he/she intends to shift the employee to the long form for that year, the employees shall be so shifted. Such notice shall state that the employee is in danger of being rated unsatisfactory in one (1) or more of the eight (8) evaluation criteria and shall state the reasons for such determination. Any employee of the short form may voluntarily shift to the long form at any time during the first semester of each year by giving written notice of such shift to his/her evaluator.
