Spoke Learning Collaborative. The Contractor will hold 5, 3 hour in-person OBOT Learning Collaboratives. The Contractor will convene a group of Vermont clinical leaders and national subject matter experts to develop a curriculum for the series. As Hub & Spoke Transition Program Protocol development has products to implement, the Contractor will build QI and outcome measures into data collection across Spoke programs. All curriculums shall be approved by the State prior to the first training. The Contractor shall supply the State the curriculum 15 business days prior to the training. The State will have 2 business days to review and provide comments back to the Contractor. Any modifications thereafter shall be subject to the State’s approval. It shall be the responsibility of the Contractor to plan, staff, and facilitate the training events including identification and recruitment of speakers and assuring quality of the presentations. The Contractor shall maintain regular communication via email and basecamp with learning community participants and post relevant documents to basecamp, assist with developing and disseminating information to Spoke Staff learning community. i. Deliverable: Completion of 5 Spoke Learning Collaboratives with CME/CEU credits issued to all participants. Two (2) Learning Collaboratives shall be completed by December 30th, 2014 and three (3) by June 30th, 2015. Any Learning Collaborative activity shall be captured on the monthly progress report submitted with monthly invoices (Appendix 1).

  • Collaboration activities 4.1 The Collaboration Suppliers will perform the Collaboration Activities and all other obligations of this Agreement in accordance with the Detailed Collaboration Plan. 4.2 The Collaboration Suppliers will provide all additional cooperation and assistance as is reasonably required by the Buyer to ensure the continuous delivery of the services under the Call-Off Contract. 4.3 The Collaboration Suppliers will ensure that their respective subcontractors provide all cooperation and assistance as set out in the Detailed Collaboration Plan.
