In order to provide the Union with timely and accurate information, the Employer agrees to furnish to the appropriate seiu union reports containing pertinent information on bargaining unit employees. Within 15 days following the preceding month the Employer will provide a report on the following information: State of addus operation Branch name Employee number Employee last name Employee first name Address City State Zip Phone number Cell phone number Email address Last hire date Termination date Termination reason code Current employment status Rate of pay Hours worked Gross pay Union dues deduction Cope/cape deduction Pac deduction Other fees (which may include voluntary deductions) Total union dues deductions Ytd hours worked Ytd gross pay Ytd union dues, Ytd cope/cape Ytd pac Ytd other fees Ytd total union dues deductions Except as specified herein, as otherwise requested by the Union or as required by law, the employer shall not release lists of employees or employee information to any third party. These monthly reports shall be securely transmitted electronically, in a commercially available format to be agreed upon by the Employer and the Union.
